some people, a positive attitude towards health improvement
and access to educational information are simply not enough
to keep them on the path to better health.
Often, one-on-one
coaching support (getting to your high risk participants
with the right messages at the right times) can make the
difference between success and struggle, gains and changes.
Mission: To affect personal lifestyle changes in
individuals who have been identified through health screenings
as having high risk for disease and multiple risk factors.
What is a health coach?
Health coaching is a “one-to-one”
targeted health intervention-education program used to enhance
the well-being of the individual and to facilitate the
achievement of their health-related goals. A health coach
assists in the behavioral change process by engaging in a
personal, trusting relationship with the participant.
A Health Coach can:

Identify risk factors for
chronic disease

Identify personal
barriers/obstacles that currently get in the way of optimal
health

Assist in adhering to
physician instructions and prescription orders

Identify desired health
outcomes and goals

Create a plan to help meet
those goals

Offer techniques to better
manage stress and anxiety

Provide the tools and training
needed for making and sustaining positive changes

Provide encouragement and
support in dealing with “bumps in the road”

Getting Started
What might the coach talk
about? The answer: whatever it takes to generate action and
produce results! The coach will push when the participant
needs to be pushed, and support when the participant is in
need of support. The coach will share strategies that will
enable the participant to deal with difficult situations and
roadblocks.
Five basic steps
1. The
high-risk participants are identified (by number of risk
factors) and enrolled in the program.
2. The
Health Coach places calls to participants’ within 30 days of the
enrollment date.
3. The
Health Coach works one-on-one with each high-risk participant to
interpret their health risks and set realistic goals for their
health behavior changes.
4. The
employer receives a quarterly activity report describing the
activity and progress of each participant.
5. At
the end of one year, all participants retake CORE programs and
re-evaluate risks and progress.
Curriculum.
Health care professionals provide information, motivation
and support for difficult lifestyle changes. Health coaching
